If you wish to make an appointment, please provide your personal details by completing our online casual patient form and include your address, DOB, phone numbers (including mobile) and NHI code (if known). It is important to also include in the final box some information about your medical concerns and related history, so that we help to organise your appointment appropriately. Once you’ve completed the form, please press the SUBMIT button and you’ll see a confirmation message that your form has been sent to Te Ora. Our administrator, Sharon, will aim to respond to you in the next week.
We can access investigations and specialist letters from those who live in the South Island. If you live elsewhere please bring any relevant medical notes, letters and results with you, or email them as attachments to firstname.lastname@example.org.
If you need to make urgent contact you can phone our administrator, Sharon, from 9-12 am on Monday to Thursday at 03 928 0922. However, the phone is not always attended at other times and messages may not be checked until the following day.
We appreciate if you are able to e-mail requests for prescriptions and supplements with 2 weeks notice before you require them by using the Contact Form or email email@example.com.
