It is helpful if you can include in the message box some information about your medical concerns and related history.
We can access investigations and specialist letters from those who live in the South Island. If you live elsewhere please bring any relevant medical notes, letters and results with you.
If you need to make urgent contact you can try phoning our nurse during the mornings at 03 928 0922. However, the phone is not always attended and messages may not be checked until the following day.
If you wish to make an appointment, please also include your personal details by completing our online casual patient form and including your address, DOB, phone numbers (including mobile) and NHI code (if known). Once you’ve completed the form, please press the SUBMIT button and you’ll see a confirmation message that your form has been sent to Te Ora. Please also see the Appointment Information and the Patient Statement of Understanding (the latter form to sign and bring with you).
With the present easing of travel restrictions we are able to provide face-to-face consultations, in addition to skype, zoom or phone consultations.
It is important that no one with possible contact with an infected person or with potential symptoms of COVID-19 come to the centre.
